COVID 19 Related Housing Considerations

According to the CDC, individuals with certain underlying conditions may have a higher risk should they be infected with COVID-19. These conditions may include, but are not limited to, diabetes or pre-diabetes, cancer, serious heart disease, lung disease including moderate to severe asthma, and being immunocompromised. 

The University had designated a limited number of single rooms in University Housing for students who identify based on CDC guidance that they are at high risk if infected with COVID-19 due to pre-existing medical conditions or being immunocompromised. The deadline for this process was June 18th, 2020 and assignments are currently in progress. Once UH has completed the review and assignment of those consideration requests, late requests will be reviewed and assigned single rooms based on availability.

In order to be considered for one of these single room options, you must follow the steps outlined below. Please pay close attention to the deadline dates for submitting the form and documentation from your medical provider.

  1. Complete the Single Room Request form by July 24, 2020.  (The deadline has passed to submit consideration requests)
  2. Submit a letter from your healthcare provider confirming that you are immunocompromised and/or your pre-existing medical condition by July 31, 2020.
    1. The letter must include your name, date of birth, and information indicating your diagnosis. The completed letter may then be faxed by your provider to Student Health Services at 1-888-972-4151. If you have a copy of the letter from your provider, you can upload it through your HealthyPackPortal. After logging into the portal, choose “Uploads”, select “Medical Records” from the drop down menu, and follow the directions to upload the letter. If you have questions concerning the submission of the letter, please call Student Health Services at 919-515-2563 for assistance. 
  3. The completed request form and the letter from your provider will be reviewed by Student Health Services.  Student Health Services may consult with other University administrators in evaluating the form, including, but not limited to University Housing.  You will be contacted if additional information is required in order to review your request.

Due to the limited number of rooms available, a completed request does not guarantee a single room assignment.
